2. What should I bring to my appointment? (Back to Questions)
All your medication bottles
Your insurance card

If you are a new patient, please bring the following:
• Names of previous physicians
• Printed and Completed Registration
• Medical History
• Privacy Forms


3. How do I get medication refills? (Back to Questions)
Don’t wait until the last pill to call the office.  If your medication bottle says you have
no refills, call about 1-2 weeks, before you run out.

If you're going out of town, plan ahead, 1-2 weeks

Refills require doctor visits.  In some cases, refills may be provided for a limited number of times.  We try to schedule an appointment before your next refill is due so you won’t be left without medicine.  You can also call for an appointment if you know your medicine is running out.

We will not authorize refills on week-ends, holidays or after business hours.

If you have a good pharmacy, the best way to get your refill is to call the pharmacy
and ask them to fax us a request for a refill.


4. What should I expect during an annual exam? (Back to Questions)
Your provider will review your health and family history and medications.  A complete exam, blood and urine tests and EKG will be performed in order to determine the need for further colon, cancer or breast cancer screenings.

Follow up appointments will be scheduled to address any health concerns or
chronic issues.

Some insurance programs such as Medicare, may not cover annual exams.  However, they are recommended and you may choose to pay out of pocket for the annual exam.


5. How will I get lab my lab and x-ray results? (Back to Questions)
We find it best to review lab results face to face during the office visit.  Therefore, we try to have these tests completed before your appointment so we may have the results prior to your visit.

However, there are times when we will send personal letters with the results and recommendations or make personal telephone calls as needed.  Therefore, it is very important that you provide us with your correct day-time phone number and address and notify us of any changes.


6. How do I obtain copies of medical records? (Back to Questions)
Requests for Medical Records must be made in writing to our office. 
Please be advised that additional charges may apply.


7. What is a Nurse Practioner?  (Back to Questions)
A nurse Practioner is a mid level provider who is qualified to treat medical problems and give prescriptions.  Commonly, they work together with physicians in caring for patients.  In our practice, it is common for our patients to see both the nurse practioner and physician during the same visit.  You may always request the provider you wish
to see.
